MRI RIGHT KNEE
TECHNIQUE AND SEQUENCES: 1.5 Tesla
Axial: FS PDW, T1W
Sagittal Oblique: PDW, FS T2W
Coronal Oblique: T1W, FS T2W, GRE T2*W
Coronal Oblique for ACL: FS T2W
FINDINGS
ACL and PCL
Complete ACL tear is anterior tibial translation and about 3.5 mm of deep lateral femoral sulcus. Pivot shift bone contusion at mid lateral femoral condyle and posterior tibial plateau as well medial femoral condyle and opposing medial tibial plateau are observed. PCL shows normal size and signal intensity
Medial Tibiofemoral Compartment
Horizontal oblique tear extending to superior surface of posterior hom of medial meniscus is noted. High grade partial tear at proximal and mid fibers of medial collateral ligament is also seen.
Articular cartilage is grossly intact.
Lateral Tibiofemoral Compartment
There is displaced posterior horm of lateral meniscus into posterior intercondylar notch with associated meniscal extrusion. LCL sprain is observed. Iliotibial band, popliteus tendon and biceps femoris tendon are preserved normal size and signal

High grade cartilagenous loss at mid lateral femoral condyle is present with subchondral marrow edema, about 1.1x1.4 cm APxTRV diameter
Patellofemoral Compartment
Patella is in normal position. Articular cartilage is grossly intact. Patchy edema of Hoffa fat pad and subcutaneous edema at medial aspect of anterior knee are noted. Quadriceps patellar tendon and patellar retinaculae are unremarkable.
Others
Moderate amount of knee joint effusion is depicted. A 1.9x0.7-cm well-defined lobulated nodule appears to be iso to slight hyperSI on T1, mixed iso and hyperSl on T2 in suprapatellar pouch, suspected a loose body. Visualized muscles and neurovascular bundles are unremarkable.
IMPRESSION:
- Complete ACL tear with anterior tibial translation, deep lateral femoral sulcus, Pivot shift bone contusion and also bone contusion at medial femoral condyle and medial tibial plateau.
-Horizontal oblique medial meniscal tear at posterior hom.
-Displaced flap tear of posterior horm of lateral meniscus into posterior intercondylar notch with meniscal extrusion. High grade partial MCL tear and LCL sprain.
-High grade cartilagenous loss at mid lateral femoral condyle, about 1.1x1.4 cm in APxTRV diameter
-Moderate knee joint effusion with a suspicious loose body in suprapatellar pouch.
