การจัดการภาวะแทรกซ้อนยาต้านรีโทรไวรัส

ตอนที่ 5 อาการไม่พึงประสงค์จากปฏิกิริยาระหว่างยาของยาต้านรีโทรไวรัส

      ปฏิกิริยาระหว่างยา (Drug interaction) หรือยาตีกันที่มีความรุนแรงและมีความสำคัญทางคลินิกมีโอกาสเกิดขึ้นได้ในระหว่างการรักษาด้วยยาต้านรีโทรไวรัส ร่วมกับยาชนิดอื่นๆ 6 กลุ่มอาการดังต่อไปนี้ถือเป็นภาวะที่รุนแรงและอันตรายที่อาจถึงแก่ชีวิตได้หากเกิดขึ้นในระหว่างที่ผู้ป่วยติดเชื้อเอชไอวีได้รับยาต้านรีโทรไวรัส คู่กับยาชนิดอื่นที่ส่งผลกระทบกับยาที่ใช้อยู่เดิม ดังนั้นผู้ป่วยที่ได้รับยาต้านรีโทรไวรัส ควรคำนึงถึงการเกิดยาตีกันเสมอ วิธีการป้องกันที่ดีที่สุด คือ ไม่ควรซื้อยา หรือหยิบยืมยาของผู้อื่นมากินเองโดยเด็ดขาด หากมีความเจ็บป่วยเกิดขึ้น ควรปรึกษาแพทย์ หรือเภสัชกรก่อนทุกครั้ง เพื่อความปลอดภัยของท่านเอง

      1. ภาวะเออโกติซึม(Ergotism) อาการของภาวะนี้ เช่น คลื่นไส้/อาเจียนรุนแรง เพลีย หน้ามืด ความดันโลหิตลดลง ชาหรือปวดที่แขนขา (โดยเฉพาะขา) อาจจะมีเขียว (cyanosis) และเกิดเนื้อเน่าตาย (gangrene)ได้ บางรายมีอาการชัก ไม่รู้สึกตัว เป็นภาวะที่มีหลอดเลือดส่วนปลายหดตัว (peripheral vascular vasoconstriction) ภาวะนี้เกิดได้จากการใช้ยากลุ่มเออร์กอต หรืออนุพันธ์ของเออร์กอต (Ergot derivative) เช่น เออโกตามีน (ergotamine) ซึ่งเป็นยาที่รักษาปวดศีรษะไมเกรน การใช้ยากลุ่มนี้ในขนาดสูงจะทำให้เกิดหลอดเลือดส่วนปลายตามที่ต่างๆหดตัวอย่างรุนแรง เช่น ที่แขนหรือขา ทำให้เกิดการขาดเลือด (ischemia) ได้ หรือถ้าเป็นที่สมองก็ทำให้ชักหรืออัมพาตได้

      ยากลุ่มเออร์กอตนี้จะผ่านซิพ 450 (CYP450) ที่ตับ ดังนั้น ยาที่มีผลเป็นซิพ อินฮิบิเตอร์ (CYP inhibitor) จะมีผลเพิ่มระดับยาเออร์กอตอย่างรวดเร็ว เช่น ยาคาร์ลิโทรมัยซิน (clarithromycin) ยาคีโตโคนาโซล (ketoconazole) ยาต้านรีโทรไวรัสกลุ่มเอ็นเอ็นอาร์ทีไอ ( NNRTIs) โดยเฉพาะยาเอฟฟาไวเร็นซ์ (efavirenz) และยากลุ่มโปรติเอส อินฮิบิเตอร์ (protease inhibitors (PIs)) โดยเฉพาะยาราลทิกราเวียร์ (raltigravir)

      ผู้ที่กินยาต้านรีโทรไวรัสกลุ่ม protease inhibitors (PIs) และเอฟฟาไวเร็นซ์ห้ามกินยากลุ่ม เออร์กอต หรืออนุพันธ์กลุ่มเออร์กอตโดยเด็ดขาด แม้เพียงเม็ดเดียวก็เกิดเรื่องได้ การรักษาในรายที่เป็นรุนแรงต้องให้ยาที่ขยายหลอดเลือด (vasodilator drug) และรักษาตัวในหอผู้ป่วยวิกฤต

      2. ภาวะตอร์ซาดเดอปวงต์ (Torsades de Pointes) เป็นภาวะที่หัวใจห้องล่างเต้นผิดจังหวะ (ventricular arrhythmia) ที่เกิดตามมาหลังจากมีภาวะ QT prolong ทำให้ผู้ป่วยเกิดเสียชีวิตอย่างกะทันหันได้ ยาต้านรีโทรไวรัสกลุ่มโปรติเอส อินฮิบิเตอร์ (PIs) หลายชนิด เช่น อะทาซานาเวียร์ (atazanavir) ที่มีระดับยาสูง มีรายงานทำให้เกิด QT prolong ข้อควรระวัง คือ ไม่ควรใช้ยากลุ่มที่เป็น ซิพ อินฮิบิเตอร์ (CYP inhibitor)โดยเฉพาะสูตรยาที่มีริโทนาเวียร์ (ritonavir) ร่วมกับยาที่มีโอกาสเกิด QT prolong สูง หรือเกิดหัวใจเต้นผิดจังหวะ เช่น แอสเทมมีโซล (astemizole) เทอร์เฟนนาดีน (terfenadine) ซิซาไพรด์ (cisapide) และพิโมไซด์ (pimozide) และ ยากลุ่มลดความดันที่ยับยั้งการหลั่งแคลเซียม เช่น ดิวไทอะเซม (diltiazem) รวมถึงยากลุ่มที่รักษาภาวะหัวใจเต้นผิดจังหวะ เช่น ฟลีเคไนด์ (flecainide) โพรพาฟีโนน (propafenone) อะมิโอดาโรน (amiodarone) ควินิดีน (quinidine) เป็นต้น

      3. ภาวะกล้ามเนื้อลายสลาย (Rhabdomyolysis) เป็นภาวะที่มีการสลายตัวของกล้ามเนื้อลายและมีการปล่อยสารต่างๆ ที่อยู่ในเซลล์เข้าสู่กระแสเลือด อาจรุนแรงจนทำให้เกิดภาวะไตวายเฉียบพลันได้ ดังนั้นไม่ควรใช้ยากลุ่มที่เป็นซิพ อินฮิบิเตอร์ (CYP inhibitor) โดยเฉพาะการให้ริโทรนาเวียร์ร่วมกับยาที่มีโอกาสเกิดภาวะนี้สูงที่ใช้บ่อยในผู้ติดเชื้อเอชไอวีคือ ยาลดไขมันกลุ่ม สะตาติน (statins) เช่น ซิวาสตาติน (simvastatin) ส่วนยาลดไขมันที่มีปฏิกิริยาระหว่างยาน้อยกว่าคือ พาวาสตาติน (pravastatin) หรือ โรซูวาสตาติน (rosuvastatin)

      นอกจากนี้ไม่ควรให้ยากลุ่มไฟเบรต (fibrate) ร่วมกับยากลุ่มสะตาตินในขณะที่ผู้ติดเชื้อได้รับการรักษาด้วยยาต้านไวรัส เพราะจะเพิ่มความเสี่ยงในการเกิดการสลายตัวของกล้ามเนื้อ หากมีความจำเป็นต้องใช้ยาทั้ง 2 กลุ่มนี้ร่วมกัน แนะนำให้เลือกยาพาวาสตาติน หรือ โรซูวาสตาติน และเลือกใช้ยาฟีโนไฟเบรต (fenofibrate) แทนยา เจมไฟโบรซิล (gemfibrozil)

      4. ภาวะความดันโลหิตต่ำ (symptomatic hypotension) โดยมีอาการ หน้ามืด หรือวิงเวียน โดยไม่ควรใช้ยากลุ่มที่เป็นซิพ อินฮิบิตเตอร์ (CYP inhibitor) โดยเฉพาะยาริโทรนาเวียร์ ร่วมกับยาลดความดันโลหิตกลุ่ม dihydropyridine calcium channel blockers เช่น ฟิโลดิปีน (felodipine) ไนเฟดิปีน(nifedipine) แอมโลดิปีน (amlodipine) หรือยาลดความดันกลุ่มยับยั้งตัวรับเบต้า หรือ เบต้า บล็อกเกอร์ (β-blocker) เพราะจะทำให้ยาลดความดันโลหิตเหล่านี้มีระดับยาสูงขึ้นและเกิดความดันโลหิตต่ำที่มีอาการได้

      ส่วนยาอื่นที่ทำให้เกิดอาการนี้ได้อย่างชัดเจน คือ ซิเดเนอร์ฟิว (sildenafil)ถ้าจำเป็นต้องใช้ ซิเดเนอร์ฟิวร่วมกับยาต้านไวรัสกลุ่ม boosted PIs โดยเฉพาะ โลปินาเวียร์/ริโทรนาเวียร์ (LPV/r) จะเพิ่มระดับซิเดเนอร์ฟิว ถึง 11 เท่า จึงให้ใช้ด้วยความระวัง โดยให้ขนาดยาซิเดเนอร์ฟิวต่ำ คือ 25 มิลลิกรัม ทุก 48 ชั่วโมง

      5. ภาวะง่วงซึมมากผิดปกติ (excessive sedative) เกิดจากการได้ยากลุ่มเบนโซไดอะซีปีน (benzodiazepines) เช่น การใช้ยาไมดาโซแลม (midazolam) ไดอะซีแปม (diazepam) ร่วมกับยาต้านรีโทรไวรัส จะทำให้ยากลุ่มนี้มีระดับยาเบนโซไดอะซีปีนสูงขึ้น เกิดภาวะง่วงซึมมากเกินไป โดยเฉพาะผู้ติดเชื้อเอชไอวีที่มีปัญหาโรคตับบกพร่องร่วมด้วย ถ้าจำเป็นต้องใช้ยานอนหลับในผู้ป่วยที่ได้รับยาต้านรีโทรไวรัส จะแนะนำให้ใช้ยา ลอราซีแปม (lorazepam) ทดแทน เนื่องจาก ยาลอราซีแปมไม่ได้ถูกขจัดออกจากร่างกายโดยเอนไซม์ไซโตโครมพี-450 (CYP450)

      6. กลุ่มอาการคุชชิง (Cushing’s syndrome) มีรายงานการเกิดกลุ่มอาการคุชชิงและการทำงานของต่อมหมวกไตทำงานไม่เพียงพอ (adrenal insufficiency) เมื่อให้ยาต้านไวรัสที่มี ยาริโทรนาเวียร์ (ritonavir) ในสูตรยา HARRT ร่วมกับยาพ่นจมูกลดการอักเสบ (ฟูติคาโซน (fluticasone)) เนื่องจาก ริโทรนาเวียร์มีคุณสมบัติเป็นซิพอินฮิบิเตอร์ (CYP inhibitor) ซึ่งอาจทำให้ระดับยากลุ่มสเตียรอยด์ซึ่งถูกทำลายผ่านด้วยเอนไซม์ไซโตโครมพี-450 (CYP450) ลดน้อยลง เนื่องจากเกิดการยังยั้งการขจัดยากลุ่มสเตียรอยด์ออกจากร่างกาย
