การจัดการภาวะแทรกซ้อนยาต้านรีโทรไวรัส

ตอนที่ 3 ยารักษาการติดเชื้อเอชไอวีในปัจจุบัน

      แนวทางการตรวจรักษาและป้องกันการติดเชื้อเอชไอวี ประเทศไทย ปี 2560 แนะนำให้เริ่มการรักษาด้วยยาต้านรีโทรไวรัสในผู้ติดเชื้อเอชไอวีทุกรายในทุกระดับ CD4 โดยผู้ป่วยที่มี CD4 มากกว่า 500 เซลล์/ลูกบาศ์กมิลลิเมตร และได้รับการรักษาด้วยยาต้านรีโทรไวรัสจะยิ่งได้รับประโยชน์จากอัตราการเกิดความเจ็บป่วยที่รุนแรงที่มีความสัมพันธ์และไม่มีความสัมพันธ์กับการติดเชื้อเอชไอวีที่ลดน้อยลง ได้แก่ การติดเชื้อวัณโรค มะเร็งชนิดคาโปซิซาร์โคมา (Kaposi sarcoma) และมะเร็งต่อมน้ำเหลือง (malignant lymphoma) กล่าวโดยรวมแล้ว การให้ยาต้านรีโทรไวรัสเร็วมีแนวโน้มที่จะลดอัตราการเสียชีวิตลงได้ ซึ่งคำแนะนำดังกล่าวของประเทศไทยสอดคล้องกับผลลัพธ์จากการศึกษาขนาดใหญ่ 2 การศึกษาที่ศึกษาในผู้ป่วยติดเชื้อเอชไอวีในต่างประเทศ (ปี พ.ศ. 2558) คือ การศึกษาที่มีชื่อว่า START study และ TEMPRANO study ซึ่งรายงานประโยชน์ในการเริ่มต้นการรักษาด้วยยาต้านรีโทรไวรัสอย่างรวดเร็วในผู้ป่วยติดเชื้อเอชไอวีในทุกระดับ CD4 ที่ตรวจพบ ประโยชน์ที่พบจากการเริ่มยาต้านรีโทรไวรัสเร็วนั้นจะช่วยลดโอกาสการถ่ายทอดหรือแพร่กระจายเชื้อเอชไอวีได้ไปยังผู้อื่นได้ ดังนั้นจึงแนะนำให้เริ่มยาต้านเอชไอวีในผู้ติดเชื้อเอชไอวีทุกรายที่มีความพร้อมและเข้าใจของการกินยาต้านรีโทรไวรัสในระยะยาว โดยก่อนเริ่มการรักษาด้วยยาต้านรีโทรไวรัส ผู้ป่วยติดเชื้อที่จะเริ่มยาต้านไวรัสต้องเข้าใจถึงประโยชน์และผลข้างเคียงของการรักษา เข้าใจประเด็นความสำคัญของการให้ความร่วมมือในการรักษาและรับประทานยา (Adherence) โดยยินดีที่จะเริ่มยาต้านไวรัสและมีความมุ่งมั่นตั้งใจรับยาต้านไวรัสอย่างสม่ำเสมอ

      ภาวะแทรกซ้อนที่สัมพันธ์กับการใช้ยาต้านรีโทรไวรัสมีโอกาสเกิดขึ้นได้กับผู้ป่วยติดเชื้อเอชไอวีทุกรายที่กำลังได้รับยาต้านรีโทรไวรัสอยู่ โดยสามารถเกิดขึ้นกับผู้ป่วยทั้งในช่วงระยะเวลาสั้น และระยะยาวภายหลังที่ได้รับยา ยาที่อยู่ในกลุ่มที่มีการทำงาน หรือการออกฤทธิ์แบบเดียวกันก็มักจะก่อให้เกิดอาการไม่พึงประสงค์ รวมถึงภาวะแทรกซ้อนจากยาในทำนองเดียวกัน ดังนั้น การเลือกใช้ยาภายหลังเกิดอาการไม่พึงประสงค์หรือแทรกซ้อน แพทย์อาจจำเป็นต้องเปลี่ยนกลุ่มยา และพิจารณาถึงความเหมาะสมอื่นๆอีกหลายประการร่วมด้วยเช่นกัน

      ปัจจุบันยาต้านรีโทรไวรัสที่ได้รับการรับรองจากองค์การอาหารและยาประเทศสหรัฐอเมริกา และนำมาใช้ทางคลินิกทั้งหมดมากกว่า 30 ชนิดขึ้นไป รูปแบบที่มีจำหน่ายมีทั้งรูปแบบยาเม็ด แคปซูล ยาน้ำสำหรับรับประทาน โดยมีทั้งยาต้านรีโทรไวรัสชนิดยาเดี่ยวและยาสูตรผสมที่ประกอบด้วยยา 2 – 3 ชนิดใน 1 เม็ด ทั้งนี้การแบ่งประเภทของยาต้านรีโทรไวรัส สามารถแบ่งตามกลไกการออกฤทธิ์หลักของยาต้านรีโทรไวรัสได้ 4 กลุ่ม ดังนี้

      1. ยาที่มีกลไกยับยั้งการเกาะจับและเข้าสู่เซลล์เป้าหมาย (Entry and Fusion Inhibitors) เช่น Enfavirtide, Maraviroc, Cenicriviroc

      2. ยาที่มีกลไกยับยั้งกระบวนการ Reverse Transcription (Reverse Transcriptase Inhibitors) ซึ่ง

แบ่งเป็น 2 กลุ่ม คือ

      2.1 Nucleoside/ Nucleotide Reverse Transcriptase Inhibitors (NRTIs) เช่น Zidovudine, Didanosine, Zalcitabine, Stavudine, Lamivudine, Abacavir, Tenofovir, Emtricitabine

      2.2 Non- Nucleoside Reverse Transcriptase Inhibitor (NNRTIs) เช่น Nevirapine, Delavudine, Efavirenz, Etravirine, Rilpivirine

      3. ยาที่มีกลไกยับยั้งกระบวนการ Integration (Integrase inhibitor, INSTs) เช่น Raltegravir, Dolutegravir, Elvitegravir

      4. ยาที่มีกลไกยับยั้งเอนไซม์ Protease (Protease Inhibitors, PIs) เช่น Sequinavir, Ritonavir, Indinavir, Nelfinavir, Amprenavir, Fosamprenavir, Lopinavir, Atazanavir, Tipranavir, Darunavir
