การจัดการภาวะแทรกซ้อนยาต้านรีโทรไวรัส

ตอนที่ 23 ภาวะพิษต่อไตเนื่องจากยาต้านรีโทรไวรัส

      

สาเหตุ

      ยากลุ่มเอ็นอาร์ทีไอ (NRTIs) ที่ทำให้เกิดภาวะพิษไตบ่อยที่สุด คือ ทีโนโฟเวียร์ (tenofovir disoproxil fumarate; TDF) มักเกิดภายหลังได้ยามานานหลายสัปดาห์ถึงนานหลายเดือน แต่บางรายเกิดเร็วในเวลา 1 – 2 สัปดาห์ โดยเฉพาะผู้ที่มีปัจจัยเสี่ยง เช่น มีค่าครีเอตินินสูงก่อนได้รับยา หรือเป็นเบาหวาน โดยความเป็นพิษนี้แบ่งออกเป็น 2 กลุ่ม คือ 1. ความผิดปกติของท่อไตส่วนต้น (proximal renal tubulopathy) และมีการเพิ่มขึ้นของค่าครีเอตินินร่วมกับมีอัตราการกรองของไตลดลง 2. การขจัดครีเอตินินลดลง โดยความผิดปกติของท่อไตส่วนต้น (renal tubular toxicity) จะมีอาการแสดง คือ ไตวายเฉียบพลัน กลุ่มอาการฟาโคนี (Fanconi syndrome) อาการเบาจืด (nephrogenic diabetes insipidus) หรือ โรคไตเรื้อรัง โดยคาดว่ามีพยาธิกำเนิดจาก การลดการขับออกของ ยาทีโนโนโฟเวียร์ (TDF) ที่ท่อไตส่วนต้นจากความผิดปกติของยีนที่ควบคุมการทำงานของตัวขนส่ง (transporters) ทำให้เกิดการสะสมของตัวยาจึงส่งผลให้เกิดความเป็นพิษต่อไมโทคอนเดรียที่บริเวณท่อไตส่วนต้น ส่วนมากผู้ติดเชื้อเอชไอวีที่มีภาวะพิษต่อไตมักไม่แสดงอาการ บางรายอาจมีปัสสาวะบ่อย มีค่าครีเอตินินในซีรั่มเพิ่มขึ้น ฟอสเฟตในเลือดต่ำ โปแตสเซียมในเลือดต่ำ และเลือดเป็นกรด (metabolic acidosis) ชนิด gap ปกติ และอาจตรวจปัสสาวะแล้วพบโปรตีน ฟอสเฟต หรือน้ำตาลได้

      เกศรินทร์ ชัยสิริ และคณะ ศึกษาอัตราการทำงานของไตในผู้ป่วยติดเชื้อเอชไอวีชาวไทยที่ได้รับยา tenofovir disoproxil fumarate (TDF) พบอัตราการทำงานของไตลดลง (ร้อยละ 25) ค่ามัธยฐานของระยะเวลาที่มีการทำงานของไตลดลง เท่ากับ 16 เดือนหลังเริ่มยา ปัจจัยเสี่ยงของการเกิดอัตราการกรองของไตลดลงร้อยละ 25 คือ น้ำหนักตัวน้อย ดัชนีมวลกาย(BMI) น้อย อัตราการกรองก่อนเริ่มได้รับยา การได้รับยากลุ่มโปรติเอส อินฮิบิเตอร์ (PIs) ร่วม และได้รับยาอื่นที่เป็นพิษต่อไตร่วม

      ยากลุ่ม INSTIs ชนิดใหม่ คือ เอวิทิกราเวียร์ (elvitegravir) ซึ่งรวมเม็ดกับยาโคบิซิสแตท/ ทีโนโฟเวียร์/เอ็มตริไซตาบีน (cobicistat/tenofovir disproxil fumarate/emtricitabine) ทำให้เกิดภาวะเป็นพิษต่อไตได้ แต่ยาที่ทำให้เกิดปัญหาหลัก คือ ยาโคบิซิสแตท (cobicistat) ทำให้การขจัดครีเอตินินลดลง เนื่องจากการยับยั้งตัวขนส่งที่ท่อไตส่วนต้น จึงพบว่าค่าครีเอตินินในซีรั่มจะสูงขึ้น แต่อัตราการกรองของไตไม่ได้ลดลงจริง และอาจเพิ่มความเสี่ยงของการเกิดพิษต่อไตได้

การป้องกัน

      ควรมีการประเมินการทำงานของไต เช่น ค่าครีเอตินิน และตรวจปัสสาวะก่อนการเริ่มยาต้านรีโทรไวรัสในผู้ป่วยติดเชื้อเอชไอวีใหม่ทุกราย ในผู้ที่มีการทำงานของไตผิดปกติอยู่แล้ว ควรหลีกเลี่ยงยาที่อาจจะทำให้เกิดภาวะพิษต่อไต หรือหากจำเป็นต้องใช้ยา ควรติดตามการทำงานของไตอย่างใกล้ชิด แนวทางปฏิบัติของประเทศไทยปี พ.ศ. 2560 แนะนำให้ตรวจค่าครีเอตินินที่เดือนที่ 1 และ 3 และทุก 6 เดือนในช่วงแรก และตรวจวิเคราะห์ปัสสาวะ (urine analysis) ปีละ 1 ครั้งสำหรับผู้ป่วยที่มีค่า creatinine clearance มากกว่า 60 มิลลิลิตร/นาที และไม่มีภาวะโปรตีนรั่วในปัสสาวะ กรณีที่พบว่า มีการลดลงของ eGFR ร่วมกับมี proteinuria มากกว่า 1+ หรือมี normoglycemic glycosuria หรือฟอสเฟตในเลือดลดลง โดยเกิดจากการสูญเสียทางปัสสาวะ ควรพิจารณาเปลี่ยนยา TDF ไปเป็นยาอื่น กรณีที่จำเป็นต้องใช้ TDF ต่อ ให้ติดตามการทำงานอย่างใกล้ชิดทุก 1 เดือน ถ้าพบความผิดปรกติมากขึ้นให้เปลี่ยนยา TDF ไปเป็นสูตรอื่นทันที

      หากผู้ป่วยที่มีค่า creatinine clearance น้อยกว่า 60 มิลลิลิตร/นาที และมีภาวะโปรตีนรั่วในปัสสาวมากกว่า 1+ จะไม่แนะนำให้เริ่มใช้ยา TDF ตั้งแต่แรก แต่หากมีความจำเป็น ให้ปรับลดขนาดยาที่เหมาะสมตามการทำงานของไต หลังจากเริ่มยาควรติดตามการทำงานของไตที่ 1-2 สัปดาห์ และ ทุก 1-3 เดือน โดยตรวจค่า serum creatinine ตรวจวิเคราะห์ปัสสาวะ ระดับฟอสเฟตในเลือดหรือปัสสาวะ กรณีที่พบว่า มีการลดลงของค่าอัตราการกรองของไต หรือมีภาวะโปรตีนรั่วในปัสสาวะมากกว่าเดิม หรือมี normoglycemic glycosuria หรือระดับฟอสเฟตในเลือดลดลงโดยเกิดจากการสูญเสียฟอสเฟตทางปัสสาวะ

      การประเมินการทำงานของไตนั้น แนะนำให้คำนวณอัตราการกรองของไตและหรือการขจัดครีเอตินิน (creatinine clearance) เนื่องจาก ค่าครีเอตินินเพียงอย่างเดียวไม่ไวพอในการวินิจฉัยว่าผู้ติดเชื้อเริ่มการทำงานของไตบกพร่อง โดยควรตรวจติดตามพิเศษเพิ่มเติม ได้แก่ ระดับฟอสเฟตในเลือด สัดส่วนการขับฟอสเฟตออก(fractional excretion of phosphate) สัดส่วนของโปรตีนต่อครีเอตินินในปัสสาวะ (urine protein/creatinine ratio) และน้ำตาลในปัสสาวะ (urine glucose) การป้องกันด้วยวิธีอื่นๆ คือ การดื่มน้ำมากกว่า 1.5 – 2 ลิตรต่อวัน และพยายามหลีกเลี่ยงการใช้ยาอื่นๆที่มีพิษต่อไตร่วม

      ทั้งนี้หากผู้ป่วยได้รับยาต้านรีโทรไวรัสอยู่ แล้วเกิดอาการไม่พึงประสงค์ดังกล่าวข้างต้น หรือมีความผิดปกติอื่นๆ ควรรีบไปพบแพทย์โดยทันที

การรักษา

      หยุดยาต้านรีโทรไวรัสที่เป็นสาเหตุ และเปลี่ยนเป็นยาอื่นๆ พิจารณาเปลี่ยนยา ทีโนโฟเวียร์ ( tenofovir disoproxil fumarate; TDF) เป็นยาอะบาคาเวียร์ (abacavir; ABC) โดยเฉพาะถ้ามีอัตราการกรองของไตลดลง โดยที่ไม่มีสาเหตุอื่นๆ มีระดับฟอสเฟตในเลือดต่ำโดยไม่มีสาเหตุอื่น หรือมีกระดูกบางหรือกระดูกพรุนร่วมกับมีการขับฟอสเฟตในปัสสาวะเพิ่ม หรืออาจเปลี่ยนยา TDF ไปเป็น tenofovir alafenamide (TAF) สามารถทำให้ภาวะโปรตีนรั่วในปัสสาวะ (proteinuria) และค่าการทำงานของไตต่างๆดีขึ้น แต่การใช้ TAF เป็นระยะเวลานานในผู้ป่วยที่มีประวัติโรคไต ควรพิจารณาติดตามการทำงานของไตใกล้ชิด และห้ามใช้ยาเมื่อค่าการกำจัดครีอะตินีน (creatinine clearance) น้อยกว่า 15 มิลลิลิตร/นาที

      กรณีที่ยังจำเป็นต้องใช้ยา TDF ต่อไป ควรปรับขนาดยา tenofovir disoproxil fumarate (TDF) ตามค่าการทำงานของไตกรณีที่จำเป็นต้องใช้ยาต่อไป การรักษาอื่นๆ ได้แก่ ให้เกลือแร่ทดแทนหรือฟอกเลือดหรือล้างไตทางช่องท้อง ปรับเปลี่ยนการดำเนินชีวิต ได้แก่ หยุดสูบบุหรี่ ควบคุมน้ำหนักและอาหาร นอกจากนี้ยังรวมไปถึงการควบคุมรักษาโรคเบาหวานและไขมันในเลือดสูง

      ในกรณีที่มีโรคไตเรื้อรังแล้ว ผู้ติดเชื้อควรได้รับการป้องกันเพื่อชะลอการดำเนินของโรค ซึ่งได้แก่ การให้ยากลุ่ม angiotensin coverting enzyme inhibitors หรือ angiotensin-II antagonist เพื่อควบคุมความดันโลหิตให้น้อยกว่า 130/80 มม.ปรอท หรือลดโปรตีนในปัสสาวะและส่งปรึกษาแพทย์เฉพาะทางโรคไตถ้ามีอาการหรือผลการตรวจทางห้องปฏิบัติการไม่ดีขึ้น
