การจัดการภาวะแทรกซ้อนยาต้านรีโทรไวรัส

ตอนที่ 12 ภาวะดื้ออินซูลินและเบาหวานเนื่องจากยาต้านรีโทรไวรัส

สาเหตุ

      ภาวะดื้ออินซูลิน คือ มีความบกพร่องในการนำกลูโคสเข้าเซลล์กล้ามเนื้อและมีการยับยั้งการสร้างกลูโคสที่ตับ หรือหมายถึงเซลล์ต่างๆที่เป็นเซลล์เป้าหมายของอินซูลินไม่ตอบสนองต่ออินซูลิน ส่งผลให้มีการเพิ่มการสร้างอินซูลินจากตับอ่อนพบได้ร้อยละ 30 – 90 ในผู้ป่วยที่ได้รับยากลุ่มโปรติเอส อินฮบิเตอร์ (PIs) แต่การเกิดเบาหวานจากยาต้านรีโทรไวรัสพบไม่บ่อยประมาณร้อยละ 7 ที่ 5 ปี ความสำคัญของภาวะดื้ออินซูลิน คือ การเป็นปัจจัยเสี่ยงของการเกิดหลอดเลือดแดงแข็ง โดยเฉพาะถ้ามีไขมันในเลือดสูงและความดันโลหิตสูงร่วมด้วย ยากลุ่มเอ็นอาร์ทีไอ (NRTIs) ที่มีผลทำให้เกิดเบาหวานหรือภาวะดื้ออินซูลินได้ คือ ยาซิโดวูดีน(zidovudine; AZT) ดีดาโนซีน (didanosine; ddI) และ สตาวูดีน (stavudine; d4T) ยากลุ่ม PIs ที่ทำให้เกิดเบาหวานหรือภาวะดื้ออินซูลิน คือ อินดินาเวียร์ (indinavir) และ โลปินาเวียร์/ริโทรนาเวียร์ (lopinavir/ritonavir) อาการของเบาหวาน คือ ปัสสาวะมาก ดื่มน้ำมาก รับประทานมาก และอ่อนเพลีย ปัจจัยเสี่ยง คือ มีน้ำตาลในเลือดสูงก่อนที่จะเริ่มยาต้านรีโทรไวรัสและมีประวัติเป็นเบาหวานในครอบครัว

การวินิจฉัย

      การวินิจฉัยเบาหวานในผู้ติดเชื้อเอชไอวีเหมือนกับผู้ป่วยที่ไม่ได้ติดเชื้อเอชไอวี ดังนี้

            ระดับ provisional

            มีอาการของเบาหวาน เช่น ปัสสาวะบ่อย กระหายน้ำบ่อย อ่อนเพลีย รับประทานมากแต่น้ำหนักลด หรืออาการที่สงสัยว่าเกิดจากภาวะแทรกซ้อนของเบาหวาน เช่น ตามัว มือเท้าชา แผลหายช้า ติดเชื้อทางเดินปัสสาวะหรือผิวหนังบ่อย เชื้อราในช่องคลอด เสื่อมสมรรถภาพทางเพศ เป็นต้น

            ระดับ probable โดยพบข้อใดข้อหนึ่งต่อไปนี้

      1. ค่าน้ำตาลหลังอดอาหาร (fasting plasma glucose (FPG)) มีค่ามากก่า 126 mg/dL โดยอดอาหารอย่างน้อย 8 ชั่วโมง

      2. ค่าน้ำตาลในเลือด (plasma glucose) ที่ 2 ชั่วโมงหลังรับประทานน้ำตาล 75 กรัม (oral glucose tolerance test (OGTT)) มีค่ามากกว่า 200 mg/dL

      3. ค่าน้ำตาลในเลือดแบบสุ่ม (random plasma glucose) มีค่ามากกว่า 200 mg/dL ร่วมกับมีอาการของเบาหวาน

      4. ค่าน้ำตาลสะสม (HbA1c) มีค่ามากกว่า 6.5% ในผู้ใหญ่

การป้องกัน

      เลี่ยงการใช้ยาต้านรีโทรไวรัสที่อาจเป็นสาเหตุ โดยเฉพาะผู้ที่มีความเสี่ยงต่อการเกิดภาวะดื้ออินซูลินและเบาหวาน ควรตรวจระดับน้ำตาลหลังอดอาหารก่อนที่จะเริ่มยาและอย่างน้อยทุก 6 เดือนหลังจากเริ่มยาต้านรีโทรไวรัส แนวทางปฏิบัติของประเทศไทยปี พ.ศ. 2557 แนะนำให้ตรวจระดับน้ำตาลหลังอดอาหารเมื่อได้รับการวินิจฉัยว่าติดเชื้อเอชไอวีในกลุ่มเสี่ยงหรือมีโรคประจำตัว หลังจากนั้นแนะนำให้ตรวจปีละครั้งหรือเมื่อมีข้อบ่งใช้ในผู้ที่มีอายุน้อยกว่า 35 ปี แต่มีโรคประจำตัว หรืออายุ 35 ปีขึ้นไป แนะนำให้ตรวจปีละ 1 – 2 ครั้ง

      หากผู้ป่วยได้รับยาต้านรีโทรไวรัสอยู่ แล้วเกิดอาการไม่พึงประสงค์ดังกล่าวข้างต้น หรือมีความผิดปกติอื่นๆ ควรรีบไปพบแพทย์โดยทันที เพื่อให้ได้รับการรักษาอย่างรวดเร็ว

      

การรักษา

      แนะนำให้ควบคุมอาหารโดยเฉพาะแป้งและน้ำตาล และให้ออกกำลังกาย การรักษาด้วยยาลดน้ำตาลในเลือดมีหลักการและเกณฑ์เหมือนผู้ป่วยที่ไม่ติดเชื้อเอชไอวี

      แนวทางปฏิบัติของการรักกษาผู้ติดเชื้อเอชไอวีที่เป็นเบาหวานของสมาคมเอดส์คลินิกแห่งยุโรป แนะนำการใช้ยารักษาโรคเบาหวานชนิดที่ 2 ด้วยยาเมทฟอมิน (metformin) และยากลุ่มซัลโฟนิวยูเรีย ( sulfonylurea) ทั้งนี้ เมทฟอมินอาจทำให้ภาวะไขมันฝ่อแย่ลง พิจารณา ยากลุ่มซัลโฟนิวยูเรียในผู้ติดเชื้อเอชไอวีที่มีระดับกลูโคสในพลาสมาสูงมากและมีน้ำหนักตัวไม่มาก โดยมีเป้าหมายเพื่อให้ระดับน้ำตาลอยู่ในช่วง 6.5 – 7.0 หากระดับน้ำตาลไม่อยู่ในช่วงสามารถใช้ยาทั้ง 2 กลุ่มร่วมกันได้ ในกรณีที่ยังไม่สามารถควบคุมน้ำตาลสะสมได้ หรืออาจส่งแพทย์ผู้เชี่ยวชาญเฉพาะทาง เนื่องจากข้อมูลเรื่องยา อาการไม่พึงประสงค์ และปฏิกิริยาระหว่างยาของยาลดระดับน้ำตาลชนิดรับประทานมีจำกัดในผู้ป่วยติดเชื้อเอชไอวี
