การจัดการภาวะแทรกซ้อนยาต้านรีโทรไวรัส

ตอนที่ 11 โรคหลอดเลือดและหัวใจเนื่องจากยาต้านรีโทรไวรัส

สาเหตุ

      จากการศึกษาส่วนใหญ่เกี่ยวกับการเกิดโรคหลอดเลือดและโรคหัวใจในผู้ป่วยติดเชื้อเอชไอวีในต่างประเทศ พบว่าอัตราการเกิดโรคหลอดเลือดหัวใจในผู้ป่วยติดเชื้อเอชไอวีสูงกว่าในผู้ป่วยที่ไม่ติดเชื้อ แต่ยังไม่มีข้อมูลในประเทศไทยหรือในเอเชียที่อาจจะมีปัจจัยเสี่ยงของโรคหลอดเลือดและหัวใจที่แตกต่างจากผู้ป่วยกลุ่มนี้ที่อยู่ในแถบตะวันตกก็เป็นไปได้ โรคหลอดเลือดหัวใจที่พบในผู้ป่วยติดเชื้อเอชไอวีและได้รับยาต้านรีโทรไวรัสนั้น อาจเป็นสาเหตุนานหลายปี โดยยาต้านรีโทรไวรัสนั้นจะทำให้เกิดภาวะดื้ออินซูลิน และไขมันในเลือดสูง ซึ่งเป็นสาเหตุหลอดเลือดแดงแข็ง (atherosclerosis) แต่การเกิดโรคหลอดเลือดหัวใจอาจเป็นจากปัจจัยอื่นๆ ได้แก่ การติดเชื้อเอชไอวีเองที่ทำให้เกิดหลอดเลือดแดงแข็ง การแบ่งตัวของเชื้อไวรัสที่กระตุ้นภูมิคุ้มกันและขบวนการอักเสบในร่างกาย ซึ่งก็จะมีผลต่อการทำงานของเยื่อบุผนังหลอดดเลือด (endothelium) รวมถึงภาวะดื้ออินซูลินจะทำให้เกิดหลอดเลือดแดงแข็ง หรือปัจจัยอื่นๆที่ไม่เกี่ยวข้องกับการติดเชื้อเอชไอวี เช่น อายุมาก สูบบุหรี่ ความดันโลหิตสูง ไขมันในเลือดสูง เบาหวานหรือปัจจัยทางพันธุกรรม ประวัติโรคหลอดเลือดหัวใจ หรือประวัติครอบครัวเป็นโรคหลอดเลือดหัวใจเมื่ออายุน้อย ซึ่งล้วนแต่มีผลต่อการเกิดหลอดเลือดแดงแข็งได้

      ยากลุ่มเอ็นอาร์ทีไอ (NRTIs) มีข้อมูลว่าสัมพันธ์กับการเกิดโรคหลอดเลือดหัวใจ คือยาอะบาคาเวียร์ (abacavir; ABC) และ ดีดาโนซีน (didanosine; ddI) โดยสัมพันธ์กับความเสี่ยงของการเกิดกล้ามเนื้อหัวใจตาย (myocardial infarction, MI) เพิ่มขึ้นและพบความเสี่ยงเพิ่มมากที่สุดในผู้ที่มีปัจจัยเสี่ยงของการเกิดโรคหลอดเลือดหัวใจอยู่แล้ว เช่น เพศชาย มีโรคเบาหวาน หรือไขมันในเลือดสูง เป็นต้น

      ยากลุ่มโปรติเอส อินฮิบิเตอร์ (PIs) สัมพันธ์กับการเกิดกล้ามเนื้อหัวใจขาดเลือด โดยยากลุ่ม PIs รุ่นแรกๆ คือ อินดินาเวียร์ (indinavir) และ โลปินาเวียร์ (lopinavir) ร่วมกับ ริโทรนาเวียร์ (ritonavir) โดย Bavinger C และคณะ พบว่ายาอะบาคาเวียร์ และยากลุ่ม PIs เพิ่มความเสี่ยงต่อโรคกล้ามเนื้อหัวใจขาดเลือดจากข้อมูลการวิเคราะห์แบบเฝ้าสังเกต (observational study) และพบว่าความเสี่ยงจะเพิ่มขึ้นตามระยะเวลาที่ได้รับยาอินดินาเวียร์ หรือ โลปินาเวียร์ (lopinavir) ร่วมกับ ริโทรนาเวียร์ แต่ถ้าเป็นการวิเคราะห์จากการศึกษาแบบสุ่มที่มีการควบคุม (randomized controlled trials) ไม่พบว่าทั้งอะบาคาเวียร์ และยา กลุ่ม PIs เพิ่มความเสี่ยงต่อโรคหลอดเลือดหัวใจ

      นอกจากนี้ ยาอะทาซานาเวียร์ ร่วมกับ ริโทรนาเวียร์(atazanavir/ritonavir) และ โลปินาเวียร์ (lopinavir) ร่วมกับ ริโทรนาเวียร์ ทำให้เกิดคลื่นไฟฟ้าหัวใจในช่วง PR interval มีความยาวมากกว่าปกติ โดยผู้ที่มีความเสี่ยงต่อการเกิดคลื่นไฟฟ้าหัวใจผิดปกติในช่วง PR interval ยาว ได้แก่ ผู้ที่มีความผิดปกติของโครงสร้างหัวใจ มีความผิดปกติของระบบการนำสัญญาณ (conduction system) โรคกล้ามเนื้อหัวใจผิดปกติ (cardiomyopathy) โรคหัวใจขาดเลือด (ischemic heart disease) และได้รับยาร่วมที่มีผลทำให้เกิด PR interval ยาวร่วมด้วย

การป้องกัน

      การป้องกันโรคหลอดเลือดหัวใจสามารถประเมินความเสี่ยงได้ตามแผนภูมิต่อไปนี้ ตามแนวทางการตรวจรักษาและป้องกันการติดเชื้อเอชไอวีประเทศไทย ปี 2560

      นอกจากนี้ ผู้ป่วยควรป้องกันปัจจัยเสี่ยงอื่นๆที่จะมีผลต่อการเกิดโรคหลอดเลือดหัวใจด้วย เช่น การควบคุมอาหาร ลดน้ำหนักในผู้ที่มีน้ำหนักเกิน ลดการดื่มแอลกฮอล์ ออกกำลังกาย และงดสูบบุหรี่ รวมไปถึงเฝ้าระวังภาวะไขมันในเลือดสูงและน้ำตาลในเลือดสูง

      แนวทางปฏิบัติของประเทศไทยปี พ.ศ. 2560 แนะนำให้ตรวจระดับคอเลสเทอรอล ไตรกลีเซอไรด์ และระดับน้ำตาลหลังอดอาหาร (fasting blood sugar) เมื่อได้รับการวินิจฉัยว่าติดเชื้อเอชไอวีในกรณีที่กลุ่มเสี่ยงหรือมีโรคประจำตัว หลังจากเริ่มยาต้านเอชไอวีแนะนำให้ตรวจปีละครั้ง ยกเว้นผู้ที่มีอายุน้อยกว่า 35 ปีที่มีโรคประจำตัวหรืออายุมากกว่า 35 ปี แนะนำให้ตรวจปีละ1 – 2 ครั้ง

      ความผิดปกติทางเมตาบอลิก เช่น โคเลสเตอรอลและ ไตรกลีเซอร์ไรด์ที่สูงขึ้น ซึ่งสัมพันธ์กับโรคหัวใจและหลอดเลือด โดยเฉพาะในผู้ติดเชื้อเอชไอวีอาจสูงกว่าผู้ที่ไม่ติดเชื้อ เนื่องจากผลแทรกซ้อนของยาต้านรีโทรไวรัส พบว่า ยาต้านรีโทรไวรัสสูตร PIs เพิ่มความเสี่ยงในการเกิดโรคหัวใจและหลอดเลือด 1.7-2.3 เท่า ดังนั้นควรประเมินความเสี่ยงของการเกิดเรื่องหลอดเลือดหัวใจและสมองในอนาคต เพื่อวางแผนป้องกันและการรักษา เช่น การประเมินความเสี่ยงตามแนวทางของ National Cholesterol Education Program, Adult Treatment Panel III (NCEP ATP III) ความเสี่ยงของการเกิดโรคหัวใจและหลอดเลือดที่ 10 ปีตามการศึกษา Framingham เป็นต้น หลังประเมินความเสี่ยงแล้วให้พิจารณาระดับ LDL ตามแผนภูมิที่ 1 โดยผู้ป่วยที่มีความเสี่ยงสูงหรือมีโรคหัวใจและหลอดเลือดอยู่แล้ว รวมทั้งในผู้ป่วยที่ไม่มีโรคหัวใจและหลอดเลือดแต่มีความเสี่ยงที่ 10 ปี มากกว่าร้อยละ 10 และผู้ป่วยที่มีโรคเบาหวาน หรือมีโรคหลอดเลือดแข็งบริเวณอื่น ถือว่าเป็นกลุ่มที่ต้องควบคุม LDL-C ต่ำกว่า 80 มิลลิกรัม/เดซิลิตร

การรักษา

      ควรเริ่มด้วยการรักษาแบบไม่ใช้ยาเป็นอันดับแรกด้วยการควบคุมอาหารการปรับเปลี่ยนพฤติกรรม (lifestyle modification) ให้การรักษาเหมือนกับการรักษาในผู้ป่วยโรคหลอดเลือดหัวใจที่ไม่ติดเชื้อเอชไอวี นอกจากนี้ควรรักษาปัจจัยเสี่ยงอื่นๆที่มีร่วมด้วย เช่น ไขมันในเลือดสูง ความดันโลหิตสูงหรือเบาหวาน อาจจะพิจารณาเปลี่ยนสูตรยาที่มีผลทำให้เกิดโรคหลอดเลือดหัวใจน้อย เช่น เปลี่ยนจากยากลุ่ม PIs เป็นยากลุ่ม NNRTIs หรือ การใช้ยากลุ่มอื่น เช่น INSTIs หรือ CCR5 antagonist ถ้าไม่มีข้อจำกัดและข้อห้าม

      ผู้ป่วยที่มีโรคหัวใจและหลอดเลือดที่อาจจำเป็นต้องรีบพิจารณาการรักษาด้วยยาที่สามารถป้องกันการเกิดโรคหัวใจและหลอดเลือดได้ทั้งปฐมภูมิ และทุติยภูมิ (primary and secondary coronary prevention) คือ ยากลุ่มสะตาติน (statin) ดังนั้นจึงเป็นยาที่ควรเลือกเป็นชนิดแรก หากผู้ป่วยมีความเสี่ยงต่อการเกิดโรคหัวใจให้พิจารณารักษาและลดระดับ LDL ให้ได้ตามเป้าหมาย
