การจัดการภาวะแทรกซ้อนยาต้านรีโทรไวรัส

ตอนที่ 1 การรักษาการติดเชื้อเอชไอวีด้วยยาต้านรีโทรไวรัสในประเทศไทย

      สวัสดีผู้ติดตาม และสมาชิกเวปไซต์หาหมอทุกท่านครับ ผม เภสัชกรวิชญ์ภัทร ธรานนท์ รู้สึกเป็นเกียรติอย่างยิ่งที่ได้รับโอกาสที่มีค่ายิ่งจากศาสตราจารย์เกียรติคุณ แพทย์หญิง พวงทอง ไกรพิบูลย์ บรรณาธิการของเวปไซต์หาหมอที่กรุณาให้โอกาสผมได้เมาเล่าเรื่องราวเกี่ยวกับอาการไม่พึงประสงค์จากยาต้านรีโทรไวรัส สาเหตุ การป้องกัน และวิธีการจัดการกับอาการไม่พึงประสงค์ที่เกิดจากยา โดยผมตั้งใจนำเสนอเนื้อหาแบ่งเป็นตอนต่างๆ โดยสามารถติดตามบทความได้ที่ Blog ของเวปไซต์หาหมอ ได้ทุกสัปดาห์ครับ สำหรับบทความที่ท่านกำลังติดตามอยู่ตอนนี้ เป็นตอนที่ 1 ซึ่งผมจะเล่าเรื่องราวเกี่ยวกับการรักษาด้วยยาต้านรีโทรไวรัสในประเทศไทย และตอนต่อไปจะเป็นสาเหตุ ปัจจัยที่ส่งผลให้เกิดอาการไม่พึงประสงค์จากยาต้านรีโทรไวรัส การป้องกัน และการรักษา โดยจะนำเสนอผลของยาที่ทำให้เกิดความผิดปกติต่อระบบต่างๆของร่างกาย ขอให้ทุกท่านที่สนใจเรื่องการใช้ยาต้านรีโทรไวรัสอย่างปลอดภัย และมีประสิทธิภาพในการรักษาโปรดติดตามบทความของผมจาก Blog ของเวปไซต์แห่งนี้ได้ครับ

      ปัจจุบันโรคติดเชื้อเอชไอวีจัดเป็นปัญหาด้านสาธารณสุขที่สำคัญของโลก เนื่องจากเป็นโรคที่มีการแพร่กระจายได้อย่างรวดเร็ว แต่อัตราการเสียชีวิตในปัจจุบันนั้นพบว่าลดลงกว่าในอดีต โดยมีผู้เสียชีวิตจากการติดเชื้อเอชไอวีทั่วโลกในปี พ.ศ.2541 สูงถึง 1.5 ล้านราย แต่ในปี พ.ศ. 2559 ลดลงเหลือ 1 ล้านราย ทั้งนี้เนื่องมาจากประสิทธิภาพของยาต้านรีโทรไวรัสที่มีประสิทธิภาพและความปลอดภัยมากขึ้น รวมถึงความตระหนักในการดูแลตนเองของผู้ป่วย สำหรับประเทศไทย ได้ให้การดูแลรักษาผู้ป่วยติดเชื้อเอชไอวี โดยมีโครงการเข้าถึงบริการด้านยาต้านไวรัสสำหรับผู้ติดเชื้อและผู้ป่วยเอดส์ระดับชาติ ซึ่งส่งเสริมโอกาสในการเข้าถึงยาต้านรีโทรไวรัสได้ครอบคลุมทั้งประเทศ และพัฒนาการตรวจวินิจฉัย การดูแลผู้ป่วยในด้านอื่นๆร่วมกันอย่างเป็นระบบ โดยความร่วมมือจากหลายองค์กร เพื่อเป้าหมายในการยุติเอดส์ของประเทศไทย

      การรักษาโรคติดเชื้อเอชไอวีด้วยยาต้านรีโทรไวรัส (Antiretroviral) ในปัจจุบัน ผู้ป่วยจะได้รับการรักษาด้วยสูตรยาที่ประกอบด้วยยาต้านรีโทรไวรัสที่มีประสิทธิภาพสูงอย่างน้อย 3 ชนิดร่วมกัน (HAART: Highly Active Antiretroviral Therapy) ปัจจุบัน ประเทศไทยสามารถผลิตยาต้านรีโทรไวรัส 3 ชนิดในเม็ดเดียวได้และมีราคายาที่ถูกลง อย่างไรก็ตาม ผู้ป่วยติดเชื้อเอชไอวีจะต้องรับประทานยาต้านรีโทรไวรัสดังกล่าวอย่างสม่ำเสมอต่อเนื่องตลอดชีวิต เพื่อลดและควบคุมปริมาณเชื้อไวรัสเอชไอวีในกระแสเลือดให้มากที่สุดจนถึงระดับที่ไม่สามารถตรวจวัดได้ และคงระดับนั้นไว้ให้นานที่สุด เพื่อลดอัตราการเจ็บป่วยและการเสียชีวิตที่เกี่ยวข้องกับโรคเอดส์ นอกจากนี้ยังช่วยฟื้นฟูและรักษาสภาพการทำงานของระดับภูมิคุ้มกันของผู้ป่วยให้กลับคืนมาทั้งในแง่ของปริมาณและคุณภาพ รวมถึงป้องกันการแพร่กระจายของเชื้อเอชไอวี ทั้งนี้ผลลัพธ์ของการรักษาเพื่อให้ผู้ป่วยติดเชื้อเอชไอวีมีคุณภาพชีวิตที่ดีขึ้น นอกจานี้ยาสูตร HAART ยังมีข้อบ่งใช้สำหรับการป้องกันการถ่ายทอดเชื้อเอชไอวีจากแม่สู่ลูก การป้องกันการติดเชื้อเอชไอวีก่อนการสัมผัสและหลังการสัมผัสที่ไม่ใช่จากการทำงาน

      การใช้ยาต้านรีโทรไวรัสจะช่วยฟื้นฟูภูมิคุ้มกันร่างกายและควบคุมการเพิ่มจำนวนของเชื้อเอชไอวี ส่งผลทำให้ผู้ป่วยติดเชื้อไวรัสเอชไอวีมีชีวิตที่ยืนยาวมากกว่าในอดีต การติดเชื้อเอชไอวีในปัจจุบันจึงเปรียบเสมือนโรคเรื้อรังที่ผู้ป่วยจำเป็นต้องได้รับยาต้านรีโทรไวรัสเพื่อการรักษาตลอดชีวิต คำแนะนำในการเริ่มต้นใช้ยาต้านรีโทรไวรัสปัจจุบันทั้งในประเทศไทย และต่างประเทศล้วนแนะนำให้เริ่มรักษาด้วยยาต้านรีโรทรไวรัสในผู้ป่วยติดเชื้อเอชไอวีในทุกระดับของค่า CD4 ซึ่งเป็นการเริ่มต้นใช้ยาที่เร็วกว่าในอดีต ในการใช้ยานั้น ผู้ป่วยที่ได้รับยาอาจประสบกับภาวะแทรกซ้อนหรืออาการไม่พึงประสงค์ของยาต้านรีโทรไวรัสที่ได้รับได้ ทั้งภาวะแทรกซ้อนระยะสั้นและระยะยาว การติดตาม แก้ไข และป้องกันภาวะแทรกซ้อนต่างๆ โดยแพทย์ และทีมผู้ดูแลผู้ป่วย รวมถึงผู้ป่วยเองก็ควรให้ความใส่ใจในการดูแลตนเองด้วย จะช่วยทำให้การรักษาประสบผลสำเร็จ และผู้ป่วยมีคุณภาพชีวิตที่ดีขึ้น
